Who can access this service Individuals or whole families aged 6-25 (with parent/carer consent if under 16) who are:
Socially Isolated, a frequent attender to GP or A&E, children attending A&E with minor injuries or missing specialist appointments, presenting with a social problem, obesity or mild to moderate mental health need.

About this service

Children, Young People and Families Social Prescribing service provides a systemic short-term whole family intervention whereby children and their families receive a full assessment and social prescription.
It aims to intervene early where children and young people are experiencing problems with self-esteem, health and well-being.

Service provides up to 8 sessions of support to help children, young people and families access activities and services in their area .
