Social Prescribing and Outreach service is a person centred service promoting good health and wellbeing by giving people access to activities, social groups, services and advice based on their needs and interests. These might also include opportunities for arts and creativity, physical activity, learning new skills, volunteering, befriending and self-help.

GP and community based support for people who are socially isolated, with long term conditions. Mild-Moderate mental health issues, experience social issues such as financial hardship.
