More Information
Offering help for a range of challenges, we offer face-to-face or telephone support that is tailored to your individual needs.
Following an initial assessment, where our team will help understand you, your situation and the support you are looking for, we provide emotional support as well as information and advice to help you access the right community and specialist services that will empower you to achieve your aims.
Open to adults (18+) registered with a Wandsworth GP practice, we support with numerous concerns or worries, including:
- mental health,
- bereavement,
- parenting issues and concerns,
- financial hardship, housing,
- employment and benefits,
- relationship breakdown,
- loneliness,
- racism and other discrimination, and
- any other issue that is causing worry or concern.
Note: We are an early help service for those aged 18+ who are registered with a Wandsworth GP practice. We do not offer crisis intervention or specialist support. For this reason we carefully screen our referrals to ensure that we are the best service to meet an individual's needs.
Referrals can be made by:
- GPs and practice-based staff (e.g. nurses, care coordinators, social prescribers),
- other health and care professionals, including VCSE organisations, and,
- your self, as a self-referral.
Wandsworth Wellbeing Foodbank Service: Family Action Wandsworth Wellbeing Foodbank Service also offers emotional and practical support to individuals that use either Wandsworth and Earlsfield Foodbanks. Find out more by clicking here.
