Who can access this service Aged 18+ residents who are registered with any GP surgery in City and Hackney, and have had a previous cancer diagnosis.

About this service

Aimed at individuals that are both registered with a GP in City and Hackney and have a cancer diagnosis, the service develops interventions with people in the area and support the delivery of personalised care.

Macmillan Social Prescribing Service also promotes good health and well-being by linking those who seek support to local activities, social groups, services, and information based on individual interests and needs.

Our team of Macmillan Social Prescribers will work with you to co-produce a personalised care plan that looks to support you, with your interests, aims and needs woven in at every opportunity.

We offer community-based support for those who have cancer diagnosis through either self-referral or across the following City and Hackney Primary Care Network (PCN) GP surgeries:

  • Well Street Common PCN
  • Hackney Marshes PCN
  • Woodberry Down and Wetlands PCN
  • Springfield Park PCN
  • London Fields PCN
  • Shoreditch and The City PCN
  • Clissold PCN
  • Hackney Downs PCN
